Office Policies

Office Policies:

• We do accept New Medicare patients.
• If you are more than 10 minutes late for your appointment. you will need to reschedule.
• NO show fees will be applied for a follow up or same day appointments $75.00.
• NO show fee for a Physical or a New Patient appointment will be $100.00.
• Cancellations for appointments must be done within 24 hours prior to your appointment or a fee may be applied to your account.
• Any forms that need to be filled out there is a charge of $30.00.
• New Patients MUST have medical records faxed to our office at (919) 851-3608 before any appointment will be scheduled. Please include demographic information such as a phone number so we may contact you. It is YOUR responsibility to contact your insurance regarding coverage. Please arrive 15 minutes prior to your appointment.
• It is the patient’s responsibility to provide active insurance to the office.
